Empowering caregivers and strengthening community connections
Pallium’s Care Connections Program consists of a suite of resources and activities to support the millions of Canadians who are caring for their family, friends, and members of their communities. These resources are provided for free and are designed to empower every Canadian to support the caregivers in their lives and to help strengthen the important social connections within our communities.
Atlas CareMap Toolkit
A Care Map is a simple, visual diagram of the people involved in providing care to a caregiver and care recipient. It includes family, friends and health care professionals who may provide assistance or support, however big or small. A Care Map allows a person to visualize and easily understand the many different connections between the people involved in caring for someone.
Atlas CareMap Community Workshop Toolkit
The Atlas CareMap Community Workshop Toolkit is a resource for anyone who would like to run a care map workshop in their neighbourhood or community. The toolkit will help increase awareness, understanding, and use of the care map tool and provide information on how it can help caregivers and care recipients.
LEAP™ Carers
LEAP Carers is free online, interactive self-learning course that provides carers with information and resources to help them provide care to their family or friends living with a serious illness. LEAP Carers is ideal for anyone who provides care to a person with a serious illness or for anyone who wants to support a carer.
