Pallium’s Care Connections Program consists of a suite of resources and activities to support the millions of Canadians who are caring for their family, friends, and members of their communities. These resources are provided for free and are designed to empower every Canadian to better support the caregivers in their lives and to help strengthen the important social connections within our communities.
Atlas CareMap Toolkit
A Care Map is a simple, visual diagram of the people involved in providing care to a caregiver and care recipient. It includes family, friends, and health care professionals who may provide assistance or support, however big or small. A Care Map allows a person to visualize and easily understand the many different connections between the people involved in caring for someone.
The Atlas CareMap Toolkit includes 6 resources for caregivers, and anyone who wants to support a caregiver, to learn about Care Maps. This toolkit contains information on what a Care Map is, why it is important, how it can support you in your caregiving journey, how to create one, and finally, how to share it with friends, family, and health care professionals.

Social Connections in Palliative Care is a free educational resource for health care professionals to learn about the importance of social connections on patient health outcomes. The module explores the use of the Atlas CareMap as a tool to increase patients’ social connections. After completing the module, health care professionals will have a better understanding of what an Atlas CareMap is, the impact of using a map in a medical and non-medical setting, and how to integrate its use into daily practice. The goal of this module is to ultimately provide better support to patients and caregivers. The module will include a helpful conversation guide to use with patients and their caregivers, additional community resources, and a fact sheet.

LEAP Carers is an online, self-learning course that provides carers with information and resources to help them provide care to their family or friends living with a serious illness. LEAP Carers is free and ideal for anyone who provides care to a person with a serious illness or for anyone who wants to support a carer.
LEAP Carers builds on the highly successful caregiver training program from the Champlain Hospice Palliative Care Program and focuses on a series of free, online interactive modules that focus on the ‘functional’ aspect of being a caregiver. Topics include navigating the health care system, advance care planning, how to provide personal care and hygiene with dignity, end of life and medication management, caregiver communication and advocacy skills as well as self-care strategies.
The Atlas CareMap Toolkit and Atlas CareMap Community Workshop Toolkit includes content that builds on or adapts work by Atlas of Care as part of their Atlas CareMap tool. Atlas of Care has completed extensive research to create the Atlas CareMap tool and Pallium Canada is working to further scale and spread the Atlas CareMap tool by adapting content for the Canadian context and developing new value-added resources to support the use and implementation of the Atlas CareMap tool.
