Support your community

Care Connections

Caring for loved ones is a journey that many Canadians undertake, often without the resources or support they need. Pallium’s Care Connections Program is here to change that.

Empowering caregivers and strengthening community connections

Pallium’s Care Connections Program consists of a suite of resources and activities to support the millions of Canadians who are caring for their family, friends, and members of their communities. These resources are provided for free and are designed to empower every Canadian to support the caregivers in their lives and to help strengthen the important social connections within our communities.

Atlas CareMap Toolkit

A Care Map is a simple, visual diagram of the people involved in providing care to a caregiver and care recipient. It includes family, friends and health care professionals who may provide assistance or support, however big or small. A Care Map allows a person to visualize and easily understand the many different connections between the people involved in caring for someone.

Atlas CareMap Community Workshop Toolkit

The Atlas CareMap Community Workshop Toolkit is a resource for anyone who would like to run a care map workshop in their neighbourhood or community. The toolkit will help increase awareness, understanding, and use of the care map tool and provide information on how it can help caregivers and care recipients.

LEAP™ Carers

LEAP Carers is free online, interactive self-learning course that provides carers with information and resources to help them provide care to their family or friends living with a serious illness. LEAP Carers is ideal for anyone who provides care to a person with a serious illness or for anyone who wants to support a carer.

Social Connections in Palliative Care - A Module for Health Care Professionals

Social Connections in Palliative Care is a free educational resource for health care professionals to learn about the importance of social connections on patient health outcomes. The module explores the use of the Atlas CareMap as a tool to increase caregiving patients’ social connections.

The information I learned from LEAP Carers has been very valuable to me as a registered nurse. I have gained more knowledge on providing care to long-term patients and learned essential skills for supporting them as they approach end-of-life care. This course has greatly enhanced my ability to offer compassionate and comprehensive support during this critical time.

Including real-life scenarios within the modules was helpful for the learning experience because they were relatable to many people’s experiences. The scenarios bridged the gap between theory and practice, making the content more engaging and memorable.

As a health care student, the LEAP Carers course was an excellent addition to my studies. It provided crucial information that I believe is essential for informal caregivers to know.
