Delivery modes
LEAP Onboard is delivered through two self-paced online modules that can be completed anytime and take less than two hours to complete.
- Module 1: Introduces key concepts of a palliative care approach, including how care needs change over time and how different members of the care team contribute to supporting patients and families.
- Module 2: Engages learners through 10 patient-centered story-based cases drawn from everyday care settings.
- Each story-based case follows a patient across four points in time—recognizing care needs, evolving goals of care, supporting comfort and dignity, and family and team support.
- At each stage, learners answer short, discovery and reflection questions, each focused on what different members of the care team (nurse, MRP, care aide, and allied health) do in that moment.
- These shared story-based cases reinforce consistent language, aligned expectations and team collaborations.
