ATTENTION - LEAP FACILITATORS/USERS (MARCH 2011) LEAP RENEWAL HERE
LEAP and LEAP Facilitators
Since
2002, the Pallium Project has nurtured a pan-Canadian
network of academic health leaders and skilled community-practitioner
champions with enhanced skills in palliative care services. Together
these champions have collaborated in developing, testing, and refining
an evidence- and normative-based modular courseware
package. This courseware is informed by Canadian practice realities and
called Learning Essential Approaches to Palliative and End-of-Life Care
(LEAP).
LEAP has been informed by an ongoing dialogic collaboration
among academic health and community-practice leaders. Each iteration
integrates current evidence with an analyses of
quality, competent practice in Canadian community settings at the
primary-care .pdf (51 KB) and
the regional resource leader .pdf (49 KB)
levels. LEAP is part of a workplace learning
approach that has been pioneered in Canada over the last decade .pdf (303 KB).
LEAP Version 1.1 is available in English and French languages in a
2-disk format (CHPCA Item 619E, 619F) and a full-color, 'off-the-shelf'
Facilitator Kit (CHPCA Item 618E, 618F) at the CHPCA Marketplace.
Alberta HOPE Project has recently published two documents that may be helpful in other Canadian jurisdictions:
- Alberta HOPE Project - 2011 Final Report (with LEAP Eval) (.pdf, 2.25 MB)
- Alberta HOPE Project - LEAP workshop guide (to 2011) (.pdf, 682 KB)
For facilitators and local leaders who have requested an interim tool to better explain
LEAP to management teams and colleagues, we have published the
Retreat Weekend Course segment of the Phase II participatory evaluation
video as a stand alone segment below. You can also use the written transcript .pdf (159 KB)
which explains the participatory evaluation methodology used for this
component of Phase II evaluation. LEAP Courseware goals and evolution segments appear further down.
LEAP Coach video segments @ youtube
All segments on the LEAP Coach DVD have now been uploaded to
the new PalliumCanada YouTube channel. A quick access guide to the final LEAP/LEAP Coach related video materials that are accessible via YouTube can be found here .pdf (118 KB).
V1.0 to V1.1 FACILITATOR KIT UPGRADE:
LEAP V1.0 was released in April 2005. It was widely released throughout
western Canada and to teams at Canada's 17 medical schools. It has come
to our attention that not all persons or organizations who have V1.0
(April 2005) kits are working with V1.1 (April 2006) kit inserts. You
can download the pages that you need. You may be asked for the
Blackline Master Permission document on the LEAP Facilitator Kit CDR if
you take these pages out-of-office to be color printed:
From time-to-time we get requests for ready access to the Participant Workbooks
contained on the LEAP V1.1 CDR. We are pleased to make those available
here for those who wish to forward an FTP downloadable link to their
local printing vendors. Please remember that you may be asked by your
local printing vendor for the Blackline Master Permission document on
the LEAP Facilitator Kit CDR, which extends local duplication rights of copyright material to LEAP license
holders. Local program planners can also adapt a guide for planning LEAP courses in a retreat-based format .pdf (733 KB).
In 2009, we also introduced an enhancement to support those
undergraduate health science students and other LEAP end-users who are
seeking a more 'environmental friendly' and practical option to have
their own LEAP module notes available 'on demand' on their own
netbooks, notebooks, portable storage/media devices, etc. Forms
fillable PDF versions of the
LEAP modules are available below:
- LEAP Module 01 - Forms fillable PDF of module slides .pdf (1.20 MB)
